Assessment of perinatal anxiety: diagnostic accuracy of five measures

Background
Anxiety in pregnancy and after giving birth (the perinatal period) is highly prevalent but under-recognised. Robust methods of assessing perinatal anxiety are essential for services to identify and treat women appropriately.
AimsTo determine which assessment measures are most psychometrically robust and effective at identifying women with perinatal anxiety (primary objective) and depression (secondary objective).
MethodWe conducted a prospective longitudinal cohort study of 2243 women who completed five measures of anxiety and depression (Generalized Anxiety Disorder scale (GAD) two- and seven-item versions; Whooley questions; Clinical Outcomes in Routine Evaluation (CORE-10); and Stirling Antenatal Anxiety Scale (SAAS)) during pregnancy (15 weeks, 22 weeks and 31 weeks) and after birth (6 weeks). To assess diagnostic accuracy a sample of 403 participants completed modules of the Mini-International Neuropsychiatric Interview (MINI).
ResultsThe best diagnostic accuracy for anxiety was shown by the CORE-10 and SAAS. The best diagnostic accuracy for depression was shown by the CORE-10, SAAS and Whooley questions, although the SAAS had lower specificity. The same cut-off scores for each measure were optimal for identifying anxiety or depression (SAAS ≥9; CORE-10 ≥9; Whooley ≥1). All measures were psychometrically robust, with good internal consistency, convergent validity and unidimensional factor structure.
ConclusionsThis study identified robust and effective methods of assessing perinatal anxiety and depression. We recommend using the CORE-10 or SAAS to assess perinatal anxiety and the CORE-10 or Whooley questions to assess depression. The GAD-2 and GAD-7 did not perform as well as other measures and optimal cut-offs were lower than currently recommended.
Observations of interannual and spatial variability in rift propagation in the Amery Ice Shelf, Antarctica, 2002–14

Iceberg calving and basal melting are the two primary mass loss processes from the Antarctic ice sheet, accounting for approximately equal amounts of mass loss. Basal melting under ice shelves has been increasingly well constrained in recent work, but changes in iceberg calving rates remain poorly quantified. Here we examine the processes that precede iceberg calving, and focus on initiation and propagation of ice-shelf rifts. Using satellite imagery from the Moderate Resolution Imaging Spectroradiometer (MODIS) and the Multi-angle Imaging Spectroradiometer (MISR), we monitored five active rifts on the Amery Ice Shelf, Antarctica, from 2002 to 2014. We found a strong seasonal component: propagation rates were highest during (austral) summer and nearly zero during winter. We found substantial variability in summer propagation rates, but found no evidence that the variability was correlated with large-scale environmental drivers, such as atmospheric temperature, winds or sea-ice concentration. We did find a positive correlation between large propagation events and the arrival of tsunamis in the region. The variability appears to be related to visible structural boundaries within the ice shelf, e.g. suture zones or crevasse fields. This suggests that a complete understanding of rift propagation and iceberg calving needs to consider local heterogeneities within an ice shelf.

Background
Few randomised controlled trials (RCTs) have examined potential preventive agents in high-risk community populations.
AimsTo determine whether a mental health literacy intervention, the promotion of physical activity, or folic acid plus vitamin B12 reduce depression symptoms in community-dwelling older adults with elevated psychological distress.
MethodAn RCT with a completely crossed 2 × 2 × 2 factorial design: (400 mcg/d folic acid + 100 mcg/d vitamin B12v. placebo)×(physical activity v. nutrition promotion control) × (mental health literacy v. pain information control). The initial target sample size was 2000; however, only 909 adults (60–74 years) met the study criteria. Interventions were delivered by mail with telephone calls. The main outcome was depressive symptoms on the Patient Health Questionnaire (PHQ–9) at 6 weeks, 6, 12 and 24 months. The Clinicaltrials.gov registration number is NCT00214682.
ResultsThe drop-out rate was low (13.5%) from randomisation to 24-month assessment. Neither folic acid + B12 (F(3,856) = 0.83, P = 0.476) nor physical activity (F(3,856) = 1.65, P = 0.177) reduced depressive symptoms at any time point. At 6 weeks, depressive symptoms were lower for the mental health literacy intervention compared with its control condition (t(895) = 2.04, P = 0.042).
ConclusionsMental health literacy had a transient effect on depressive symptoms. Other than this, none of the interventions significantly reduced symptoms relative to their comparator at 6 weeks or subsequently. Neither folic acid plus B12 nor physical activity were effective in reducing depressive symptoms.

The ROSAT X-ray astronomy satellite, due to be launched in early 1990, will carry two separate and complementary grazing-incidence telescopes with co-aligned axes. The German X-ray telescope (XRT) will cover the soft X-ray region in the range 0.15–2 keV (6–80 Å), while the U.K. XUV Wide Field Camera (WFC) will extend coverage to beyond 200 Å. The WFC is a joint project of Leicester and Birmingham Universities, the Mullard Space Science Laboratory, and the authors' institutes. The primary objective of ROSAT is to perform an all-sky survey over a period of six months. This will be followed by a guest-observer, “pointed” phase. We briefly discuss the sensitivity of the WFC to the soft X-ray/XUV background (SXRB) and the problems and techniques associated with distinguishing the astronomical background from other sources of background.

The ROSAT X-ray satellite mission and its X-ray telescope (XRT) are described by Trümper (1984). The characteristics of the Wide Field Camera (WFC) on ROSAT and its potential for studies of the soft X-ray background (SXRB) are discussed by Harris, Sumner, and Walker (1989, this volume). The energy range covered by the WFC is 0.06 keV to 0.21 keV (60 Å to 200 Å), whilst the XRT covers the higher energy range from 0.2 keV to 2 keV. Observations performed to date in this field have given rise to conflicting evidence on the location and nature of the 106 K gas, which is presumed to be the origin of the observed emission (see references in Harris, Sumner, and Walker, 1989, this volume).

We have constructed a detailed model for the infrared (IR) point source sky that comprises geometrically and physically realistic representations of the galactic disk, bulge, stellar halo, spiral arms (including the “local arm”), molecular ring, and the extragalactic sky. We represent each of the distinct galactic components by up to 87 types of galactic source, each fully characterized by scale heights, space densities, and absolute magnitudes at BVJHK, 12, and 25 μm, and a spectrum from the IRAS Low Resolution Spectrometer (LRS). Our model has been guided by a parallel Monte Carlo simulation of the Galaxy at 12 μm. The content of our galactic source table constitutes an excellent match to the 12 μm luminosity function in the simulation, as well as to the luminosity functions at V and K. We are able to predict differential and cumulative IR source counts for any bandpass lying fully within the IRAS LRS range (7.7–22.7 μm) as well as for the IRAS 12 and 25 μm bands. These source counts match the IRAS observations extremely well. The model can be used to predict the character of the point source sky expected for observations from future IR space experiments (e.g., ISO, SIRTF, LDR).
